There was a news story run by a few media outlets that an Air India plane (Air India Flight 126) had to return to Chicago when most of the planes toilets were put out of order within an hour of the flight.

It was claimed that investigations showed that the passengers had dumped plastic bags, rags and even clothing down the planes toilets. Apparently this was not an unusual occurrence on Air India flights, but the article didn't explain why the passengers didn't just use the bins in the toilets to dispose of the items (as instructed on the notices).
